ROMANUS LESION CORNER INFLAMMATORY LESION MRI

Romanus Lesions are now called Corner Inflammatory Lesions. This post is the last in the series on Romanus lesions in ankylosing spondylitis and looks at the MRI findings in both acute and chronic changes. WHAT HAPPENS AT THE ENDPLATES:

As we saw in the first part, the changes seen at the endplate corners are due to an enthesitis which are progressive.

1. In the acute stage inflammatory changes are present at the vertebral endplate corners (anterior > posterior)
2. As the inflammation reduces that region is replaced by fatty infiltration and at the end stage, becomes sclerotic.
